Job Description:

We seek a dedicated LPN-LVN II to join our team, working in both
facility-based and home-based settings across 42 states. This role
is essential to our mission of restoring quality of life and
promoting patient independence through attentive, compassionate
care. The ideal candidate will be responsible for patient care
under the supervision of registered nurses and physicians, ensuring
that each patient’s treatment plan is executed with precision and
compassion. Administer nursing care to ill, injured, or disabled
patients within a facility or at home. Monitor and report changes
in patient symptoms or behavior. Communicate with registered nurses
and other multidisciplinary healthcare team members to provide
continuity of care. Implement patient care plans meticulously under
the supervision of registered nurses and physicians. Operate
medical equipment and monitor for proper functioning. Assist in the
home health care of patients, providing support and education to
patients and their families. Proficiency in administering
medication and performing routine medical operations Experience
with Electronic Medical Records (EMR) systems Strong observational
skills and attention to detail Excellent communication and
interpersonal skills Medical Insurance Dental Insurance Vision
Insurance 401k Current LPN or LVN license valid in the state of
employment. CPR/BLS certification.
