Job Description:
Encompass Health Corporation is seeking a dedicated LPN-LVN II
to join our multidisciplinary team, providing high-quality,
facility-based, and home-based care at our rehabilitation and
healthcare centers. This role is pivotal in administering nursing
care under the supervision of a registered nurse or physician,
ensuring the comfort and well-being of patients through a
compassionate, evidence-based approach. As an LPN-LVN II, you will
be instrumental in providing exceptional patient experiences
aligned with Encompass Health Corporation’s standards of
excellence. Monitor and record patient’s condition and document
care provided effectively Administer prescribed medications and
simple treatments as directed by physician or registered nurse
Coordinate with healthcare team to assess, plan, implement, and
evaluate patient care plans Provide patient and family education
focused on health recovery, disease prevention, and caregiver
support Execute patient care duties with an emphasis on compassion
and efficiency Ensure compliance with all health and safety
regulations within the facility and in home care settings Maintain
patient confidentiality and respect patient rights at all times
Proficiency in administering medications and understanding of
treatment protocols Excellent patient assessment skills and ability
to handle emergencies Strong communication and interpersonal skills
Capacity to multitask and manage time effectively Familiarity with
medical software and patient care documentation systems Medical
Insurance Dental Insurance Vision Insurance 401k CPR certification
required Valid driver’s license for home-health care duties, if
applicable
