Job Description:

home-based care. Our nurses ensure effective patient care delivery
and the achievement of desired patient outcomes. This position

Provide direct and individualized nursing care to patients based on
the application of scientific nursing principles. Assist in the
assessment, planning, implementation, and evaluation of patient
care in collaboration with the interdisciplinary health team.
Document patient healthcare outcomes and respond dynamically to the
changes in condition, utilizing problem-solving skills and clinical
judgment. Educate patients and families on disease processes,
treatment plans, and healthcare maintenance post-discharge. Ensure
compliance with healthcare regulations and safety standards.
Proficiency in managing complex patient care specialties. Strong
organizational and interpersonal skills. Ability to operate medical
equipment and perform health assessments. Excellent written and
verbal communication skills. Medical Insurance Dental Insurance
Vision Insurance 401k Valid Registered Nurse (RN) license in the
state of employment. Basic Life Support (BLS) Certification.
Advanced Cardiac Life Support (ACLS) Certification preferred.
